Various Erectile dysfunction treatments

The purpose of ED treatment is to allow men to achieve and maintain long-lasting and strong erections that are essential for satisfactory sexual intercourse. There are a variety of treatments involved. These include:

  • Prescription medications
  • Three drug injection therapy (Knoxville formula)
  • Injection therapy
  • Vacuum pump therapy
  • MUSE
  • Implantable penile prostheses
  • Surgery

Three prescription medications are available for the treatment of physical causes of ED. About three-fourths of all men with ED are afforded relief with these drugs. They work to enhance the effects of nitric oxide, a chemical that relaxes muscles in the penis and allows more blood to flow in. Pills by themselves do not cause an erection; for an erection to occur, sexual stimulation is required. However, men should not have sex more than once every 24 hours after using these ED drugs. These are:

Sildenafil citrate (Viagra): It was originally developed as a treatment for angina, or chest pain. Since receiving approval as a treatment for erectile dysfunction, it has been prescribed to more than 20 million men worldwide. A vasodilator and the first oral medication for ED treatment, Viagra dilates the blood vessels, improves blood circulation and enhances the effects of nitric oxide. Nitric oxide is the agent that relaxes the smooth muscle in the penis and regulates blood vessels during sexual stimulation, leading to an erection.

The average recommended dose of Viagra is 50mg but also comes in doses of 25mg and 100mg. This drug must be taken about an hour before sex and lasts up to 4 hours in the body. However, it should be taken on an empty stomach for it to be effective. High-fat foods can also interfere with the absorption of Viagra. Viagra does not increase sexual desire.

Vardenafil (Levitra): This is a second prescription drug that treats erectile dysfunction. Like Viagra, it helps increase blood flow in men with ED so that they can get and keep an erection. Once sexual activity has been concluded, blood flow to the penis must decrease and the erection should go away. Levitra must be taken about an hour before sexual activity and last up to 4 hours in the body. Studies have revealed that Levitra improves erectile function in men who have other health factors like diabetes or prostate surgery.

Men taking nitrate drugs or alpha-blockers should not take Levitra. Combining these drugs with Levitra may cause blood pressure to drop to an unsafe level. Levitra is available in 2.5mg, 5mg, 10mg, and 20mg tablets.

Tadalafil (Cialis): Cialis is the third oral ED drug. Its most notable difference lies in the fact that it works for up to 36 hours. Cialis helps increase blood flow in the penis when a man receives sexual stimulation as well as get and keep an erection satisfactory for sexual activity. Once the sexual activity has been completed, blood flow to the penis decreases, and the erection goes away. It is clinically proven to improve erectile function in most men with all levels of ED.

The most common side effects with Cialis are headache, upset stomach, back pain, and muscle aches. These side effects generally go away within a few hours of taking the drug.

The recommended starting dose for Cialis is 10mg but also comes in 5mg and 20mg doses. Since the absorption of Cialis is not affected by food or high-fat foods, it does not need to be taken on an empty stomach. Cialis begins working in about 30 minutes and may be taken up to once per day by most patients.

Before the approval of Viagra in 1998, drug treatment of erectile dysfunction involved injecting alprostadil (prostaglandin) or inserting it as a pellet into the urethra.

Although, ED drugs have been largely useful in most men, prostate cancer patients and ED caused by psychological problems often require alternative forms of treatment. In such cases, the commonly used alternative consists of a three-drug injection containing alprostadil, papaverine hydrochloride, and phentolamine mesylate and is commonly referred to as the “Knoxville formula.” The three-drug preparation is administered by injection into the corpora cavernosa to induce an erection.

In the vacuum pump therapy, a man inserts his penis into a clear plastic cylinder and uses a pump to force the air out of the cylinder, forming a partial vacuum around the penis, which helps to draw blood into the corpora cavernosa. Then a special ring is placed over the base of the penis to trap the blood inside it. Side effect of this treatment is occasional bruising if the vacuum is left on too long.

Injection therapy involves injecting Alprostadil (Caverject) into the penis to relax smooth muscle tissue and enhance blood flow into the penis to cause an erection. It must be injected shortly before intercourse. Another, similar drug that is sometimes used is papaverine. Either drug has the potential to sometimes cause painful erections or priapism that can be treated with a shot of epinephrine. Alprostadil can also be administered into the urethral opening of the penis.

In MUSE (medical urethral system for erection), a thin tube the width of a spaghetti noodle is inserted into the urethral opening and then pressed down on a plunger to deliver a tiny pellet containing alprostadil into the penis. The drug takes about 10 minutes to work and the erection lasts for about an hour. The main side effect here is a sensation of pain and burning in the urethra, which can last from about 5-15 minutes. The injection process itself is also often painful.

Implantable penile prostheses are usually considered a last resort for treating erectile dysfunction. They must be implanted in the corpora cavernosa to make the penis rigid without the need for blood flow in. The semi-rigid type of prosthesis consists of a pair of flexible silicone rods that can be bent up or down. This device has a low failure rate but, causes the penis to be erect always, which may become difficult to conceal under one’s clothing with ease.

Whereas, the inflatable type of device is surgically implanted. It consists of cylinders that are implanted in the corpora cavernosa, a fluid reservoir is implanted in the abdomen, and a pump is placed in the scrotum. The pump is squeezed to move the fluid into the cylinders that cause them to become rigid. The process is reversed by squeezing the pump again. However, these devices allow for intermittent erections and have higher malfunction rates than the silicon rods. Men can return to sexual activity six to eight weeks after implantation surgery. This procedure permanently takes away a man’s ability to have a natural erection.
